Social Worker (MSW)

Role: Hospice Medical Social Worker Responsibilities include: Providing emotional, social, financial, and resource support to patients and families facing serious illness and end-of-life care. Key duties involve conducting social casework, assessing financial situations, facilitating referrals, participating in interdisciplinary team conferences, and connecting families with community resources. Ensuring the use of patient-centered approaches such as the 4Ms (What Matters, Medications, Mentation, Mobility) and providing age-friendly care are essential. Qualifications & Skills Master’s degree in Social Work (MSW) from an accredited program Current state Social Worker License Proficiency in clinical skills, excellent communication, and documentation abilities Ability to make sound clinical judgments and develop individualized care plans Valid driver’s license and good driving record Proficiency in computer and EMR systems Preferred Experience & Certifications Advanced Palliative Hospice Social Worker Certification (APHSW-C) Minimum two years’ hospice experience Certified in Volunteer Administration (CVA) Key Attributes: Compassion, empathy, strong assessment skills, collaboration, and commitment to providing dignified, patient-centered care in a supportive team environment. #J-18808-Ljbffr
