Work Environment

As a Pediatric Nurse Practitioner at Cherish Care Wellness, you will work primarily in a home care setting, providing direct care to our pediatric patients in the comfort of their homes. Key Responsibilities

• Conduct comprehensive assessments and establish care plans in collaboration with attending physicians, ensuring evidence-based practices are followed.
• Diagnose and treat a variety of common pediatric conditions, while coordinating with specialists as necessary to optimize patient outcomes.
• Provide direct patient care during home visits, including physical examinations, immunizations, and management of chronic health conditions.
• Educate patients and their families on health maintenance, disease prevention, and the management of medical conditions, fostering an engaging and supportive environment.
• Develop and implement an interdisciplinary plan of care that considers psychological, sociocultural, spiritual, economic, and lifestyle factors affecting the patient.
• Maintain meticulous records, ensuring all patient interactions and care plans adhere to regulatory standards and organizational policies.
• Participate in quality improvement initiatives and maintain a commitment to continuous professional growth and advanced nursing practice.
• Actively engage in interdisciplinary team meetings to share insights and improve service delivery for pediatric patients.

Patient Population / Client Focus

Your primary focus will be on children from infancy through adolescence, treating a broad spectrum of acute and chronic illnesses in a home care setting. You will work closely with families to create a supportive atmosphere that addresses each child's unique developmental needs and health challenges. Qualifications

• A Bachelor’s Degree in Nursing from an accredited institution.
• A Master’s Degree in Nursing with a focus on pediatric care.
• Current national certification as a Nurse Practitioner (NP) and Basic Life Support (BLS) certification.
• Commonly required credentials for pediatric nurse practitioners (CPNP) based on departmental guidelines.

Preferred Experience

• At least five years of relevant experience working as a Nurse Practitioner in pediatrics, with a proven track record of delivering high-quality care.
• Hands-on experience managing pediatric patients within a home care or outpatient setting.
• Strong communication skills to effectively collaborate with patients, families, and the multidisciplinary healthcare team.
