Join #TeamAcenda and make a lasting impact.To keep families together by providing safe, permanent housing, and ongoing accessibility to evidence-based trauma informed support services to Child Protection and Permanency families throughout our region.The Case Manager will provide comprehensive case management and supportive services to families experiencing homelessness or housing instability, helping them build stability and achieve long-term self-sufficiency. Partner with families to develop life skills, connect them with community resources, facilitate groups, and maintain accurate documentation to support positive outcomes.As an integrated health organization Acenda values the use of evidence-based practice or EBP's.Key Responsibilities:Provide support services and case management to families with a history of homelessness or housing instability.Assist parents to make a successful transition to self-sufficient living and improved individual and family stability.Provide coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, advocacy skillsCo-facilitate groups for parents, children and families.Link families to both formal and informal community resources.
