Compassionate Home Health RN Case Manager

Humana/CenterWell is seeking a Home Health RN Case Manager in Overland Park, KS. You will manage admissions, plan and oversee home health care, and coordinate with physicians and multidisciplinary teams to optimize patient outcomes. The role requires RN licensure, 1+ year of nursing experience, CPR, and a valid driver’s license.
