Licensed Practical Nurse (LPN)We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in a Long-Term Care/Skilled Nursing Facility (LTC/SNF) setting. This role involves providing high-quality patient care to an elderly population, including those with dementia and various behavioral diagnoses.Shift Details12-hour day shiftsEvery other weekend and holidays requiredGuaranteed 36 hours per weekResponsibilitiesAssist in the delivery of patient care under the supervision of registered nursing staff.Collaborate with the medical team to implement care plans and monitor patient progress.Administer medications and treatments as prescribed.Document patient care and charting using an Electronic Medical Record (EMR) system, specifically PCC.Ensure the comfort and safety of residents while providing compassionate support.Participate in patient and family education regarding care plans and health management.QualificationsCurrent and valid LPN license.Basic Life Support (BLS) certification required.Minimum of 1 year of experience in a Long-Term Care or Skilled Nursing setting preferred.Additional InformationOrientation and training will be provided, including opportunities to shadow experienced staff.Common patient diagnoses include a variety of conditions seen in the elderly population.Join our team and make a difference in the lives of those we care for.
