In this role, you will provide high-quality patient care, perform treatments, administer medications, and support residents' overall health and comfort. You will also collaborate with CNAs to ensure timely, accurate care and maintain thorough documentation.Key ResponsibilitiesPatient Care & SupportPromote and restore residents' health through the nursing processPerform treatments and procedures as prescribedProvide physical, educational, and emotional support to residents and familiesSupervise assigned CNA team members to ensure quality careMedication Administration & ReportingSafely administer medications according to physician ordersGive and receive relevant reports, both written and verbal, from appropriate staffDocumentation & ComplianceMaintain accurate, complete resident care documentation using electronic medical recordsEnsure CNA documentation is accurate and timelyQualificationsCurrent, valid NY LPN licenseCPR CertificationExperience in a nursing home or hospital setting preferredStrong teamwork, communication, and organizational skillsThe Grand Healthcare System operates a network of state-of-the-art rehabilitation and nursing facilities dedicated to providing advanced, compassionate care.
