Occupational Therapist

Role Overview: The Occupational Therapist provides age-appropriate rehabilitative care, addressing physical, emotional, developmental, psychosocial, and educational needs of patients in a home health setting. They collaborate with multidisciplinary teams to develop and revise personalized care plans, ensuring all goals are met. Key Responsibilities Deliver direct patient care through assessments, therapeutic exercises, education, and treatments, managing patient needs from admission to discharge. Coordinate services by developing and adjusting care plans in accordance with physician orders and patient progress. Educate patients and caregivers to promote independence and support recovery. Participate in staff training, continuing education, and professional development to stay current with clinical practices. Accurately document interventions, communicate patient status changes to physicians, and adhere to regulatory guidelines. Qualifications & Skills Graduate of an accredited Occupational Therapy program with a valid license and certifications, including BLS and driver’s license. Minimum of one year of full-time occupational therapy experience, preferably in home health. Strong assessment, rehabilitative, organizational, and communication skills; proficiency with computer systems and home health operations.
