Registered Nurse (RN) - LTC

This RN role supports residents in a long-term care facility, focusing on geriatric and chronic disease management, rehabilitation, and palliative care coordination.

The nurse contributes to care planning, safety, and transitioning residents between services while maintaining comfort and quality of life in a skilled nursing environment.

Responsibilities

• Perform comprehensive assessments and monitor chronic conditions, medications, and functional status.
• Administer prescribed medications and therapies with attention to dosing clarity and reconciliation.
• Provide wound, skin, and IV care as indicated; manage hygiene, nutrition, and mobility supports.
• Develop and update individualized care plans with input from residents, families, and the care team.
• Document care activities and changes in the EHR; communicate changes to the interdisciplinary team.
• Educate residents and families on disease management, medications, and care goals.
• Collaborate with physicians, therapy teams, and social work for discharge planning and transitions.
• Respond to clinical changes with timely assessment and escalation when needed.

Qualifications

Education:

ADN or BSN from an accredited nursing program.

Licensure:

Current RN license valid in the state of employment.

Certifications:

• Basic Life Support (BLS) certified
• Gerontologic Nursing Certification or geriatric-focused training preferred

Experience or Skills:

Experience in long-term care or skilled nursing settings; strong communication, documentation, and teamwork abilities; comfort with geriatric care needs.

Experience

• Experience in a skilled nursing facility or long-term care unit
• Experience with chronic disease management and rehabilitation support
• Experience documenting in EHR and coordinating with families and providers

Preferred

• BSN preferred
• Experience with wound care and falls prevention programs

Work Environment

Care in a skilled nursing facility with a focus on comfort, safety, and quality of life for older adults; a multidisciplinary team includes physicians, therapists, nutritionists, and social workers.

Schedules vary by facility and unit needs, with day, evening, or rotating shifts and priority on documentation accuracy and resident satisfaction.
