As a Tailored Care Manager, you'll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands-on role with the opportunity to travel throughout Goldsboro, NC and surrounding counties helping close gaps in care and improving lives in the community every day.Why Join Us?As a valued member of our team, you'll earn a competitive salary ranging from $53,000 to $56,000 annually, based on your experience and qualifications. This full-time role offers a primarily Monday–Friday schedule from 8 AM to 5 PM, with rotating on-call responsibilities. This role requires flexibility to provide services in various community settings and participate in rotating after-hours support.We also require:Bachelor's degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area.
