Physical Therapist (New Graduate Welcome) – Skilled Nursing Facility About the Role We are seeking a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team in a skilled nursing facility. New graduates are encouraged to apply! We are willing to train the right candidate and provide mentorship to help develop clinical skills and confidence in a post-acute care setting. This is an excellent opportunity to work with an interdisciplinary team dedicated to helping residents regain mobility, improve function, and enhance their quality of life. Responsibilities Evaluate residents and develop individualized treatment plans. Provide physical therapy services to residents recovering from illness, injury, surgery, or functional decline. Establish realistic goals and monitor progress toward achieving optimal outcomes. Document evaluations, treatment plans, progress notes, and discharge summaries in accordance with facility and regulatory requirements. Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals. Educate residents, families, and caregivers on treatment plans and home exercise programs. Participate in care planning meetings and interdisciplinary team discussions. Ensure compliance with all federal, state, and facility policies and regulations. Qualifications Required Graduate of an accredited Physical Therapy program. Current or eligible Physical Therapist license in the state of employment. Strong communication and interpersonal skills. Ability to work effectively in a team-oriented environment. Passion for helping older adults improve their mobility and independence. Preferred Previous clinical rotations or experience in skilled nursing, rehabilitation, geriatrics, or long-term care.
