Community Care Givers – Grand Rapids, MI
Pay: $68–$70/visit , based on Medicare home health and OASIS experience

Community Care Givers is seeking an experienced Registered Nurse (RN) to provide Medicare-certified home health services and complete Start-of-Care (SOC) admissions and OASIS assessments throughout the Grand Rapids area.

Responsibilities

• Complete SOC, ROC, recertification, transfer and discharge visits as needed

• Perform comprehensive nursing assessments

• Develop and update individualized Plans of Care

• Complete medication reconciliation and patient education

• Coordinate physician orders and communicate with the care team

• Assess skilled nursing and homebound needs

• Maintain accurate documentation to support Medicare eligibility and medical necessity

Qualifications

• Current Michigan RN license in good standing

• At least 1 year of RN experience

• Medicare home health/OASIS experience strongly preferred

• Experience with Medicare SOC admissions preferred

• Strong clinical assessment, documentation and communication skills

• Reliable transportation and valid driver’s license

• Comfortable using electronic medical records and mobile documentation

Schedule

Monday–Friday, primarily daytime hours
Some flexibility may be available based on experience and agency needs.
