Senior Case Manager: Mental Health Housing & Recovery
The Jewish Board
The Jewish Board seeks an experienced Case Manager to support adults in our supportive housing programs. You will manage complex caseloads, implement person-centered service plans, and coordinate with community partners to assist residents in achieving independent living goals. Expect to document progress, lead routine sections of care, and advocate for residents while navigating health disparities.
