Registered Nurse Telehealth

Providing telephonic and telehealth care, the part-time Home Health Telehealth Registered Nurse will manage triage assessments, case management, and patient coordination remotely on weekends from 8 AM to 5 PM EST or CST. Key responsibilities Conduct triage assessments and manage incoming patient/caregiver calls during assigned shifts Educate and follow up with patients and caregivers on chronic disease management and coordinate care with healthcare providers Document all patient interactions and participate in interdisciplinary team meetings to ensure optimal patient care Required qualifications Current RN license in the state of practice 2-3 years of clinical practice experience in a relevant field 1-3 years of home health or hospice experience Knowledge of disease management and chronic disease processes Excellent telephone communication skills
