Licensed Vocational Nurse (LVN)

As a Registered Nurse you will plan and deliver nursing care to residents in accordance with company, federal, state, and local standards, guidelines, and regulations to ensure the highest degree of quality care and dignity is maintained at all times. Responsibilities Maintain an acceptable standard of nursing practice and professional decorum. Be courteous, considerate and cooperative when communicating with all facility staff, residents and public. Promote and maintain good public relations on behalf of the facility. Possess knowledge of Federal and State Regulations governing long‑term care nursing facilities. Implement the facility’s policies and procedures. Direct and assist Licensed Nurses in clinical performance areas. Perform the functions and responsibilities of a Charge Nurse as assigned. On the day shift, perform the functions and responsibilities of Assistant Director of Nursing Services as directed. React appropriately to emergencies and disaster situations. Notify the Administrator or Director of Nursing Services of any serious situation requiring administrative direction. Perform other duties as may be assigned by the Director of Nursing Services. Other duties and responsibilities may be assigned by the administrator due to facility needs. Conduct daily physical and mental status assessments and document them accordingly. Participate in client observations to promote the abstinence of behaviors and safety. Organize, coordinate, and follow up on all consults with physicians or medical personnel for tests, and provide the appropriate diagnosis for laboratory, radiology, and other diagnostic testing. Create and maintain weight and vital signs in the EMR and check daily for accuracy; recheck any documented weight or vital sign that is unusual. Complete all Alsana Nursing Assessments, Care‑plans, S.O.A.P notes, Admission notes, Discharge notes, Noc shift notes and accurately document them in the medical record. All LVN notes must be co‑signed by the Nurse Manager and/or RN. Administer first aid according to facility policy, including all appropriate follow‑up documentation. Obtain insurance updates to get precertification for medications and other coverage. Qualifications Nursing degree from an accredited college or graduation from an approved RN program required. Current, unencumbered license to practice as an RN in California.
