Adult Case Manager: Community Outreach & Care Coordination

The Guidance Center

The Guidance Center in Taylor, Michigan is seeking a Human Services Professional to support adult mental health consumers through individualized assessments and service plans. You will be responsible for ensuring clients access community resources and maintaining their mental health needs through ongoing case management. The role requires a bachelor's degree in a human services field and involves developing collaborative relationships with consumers, families, and support systems.
