Description

• Conduct thorough assessments of patients in either inpatient or outpatient environments and create personalized treatment plans
• Implement physical therapy interventions that target mobility, strength, balance, and functional ability
• Team up with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes
• Educate patients and their families on therapeutic objectives, discharge strategy, and home exercise routines
• Accurately document evaluations, treatments, and patient progress following facility and regulatory requirements
• Engage in multidisciplinary care team rounds and conferences to ensure coordinated care practices

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (such as in neurorehabilitation or geriatrics) are advantageous but not mandatory

Skills

• Strongly preferred previous experience in a clinical setting
