In this role, you will work in a Skilled Nursing Facility setting that emphasizes high-quality, patient-centered rehabilitation services for adults and geriatric patients. You can expect to provide therapy services for patients with a range of conditions, including post-acute orthopedic, neurologic, cardiopulmonary, and general deconditioning cases. The environment supports interdisciplinary collaboration, working alongside nursing, occupational therapy, speech therapy, and other team members to develop individualized care plans. Typical responsibilities include conducting initial and ongoing PT evaluations, creating and progressing evidence-based treatment plans, providing therapeutic exercises and functional training, focusing on balance and gait, fall prevention strategies, and safe mobility. You will participate in care conferences, communicate clearly with the care team, and contribute to discharge planning to help patients safely transition to the next level of care. Accurate and timely documentation in the EMR is a key part of the role. The typical day involves managing a caseload appropriate for a SNF setting, with a focus on quality outcomes and functional progress rather than rapid turnover. Patient ratios are structured to allow for meaningful one-on-one treatment time and thorough assessments. You will work standard day shifts, 5 days per week, with 8-hour shifts, for an expected 40 hours per week. The contracted period is 13 weeks, with weekly billing and pay cycles, and no on-call responsibilities.
