LPN Admissions Representative (PRN- as needed)

LPN Admissions Representative PRN- as needed Day Shift (8am - 5pm or 9am - 6pm) Not a remote position ESSENTIAL FUNCTIONS: Conducts remote preadmission discussions with patients and families to capture and document patient story and schedules the visit based on level of complexity and urgency.
Enters nursing related information into the electronic medical record (EMR), including allergies and pertinent diagnoses Triages phone calls coming into Intake & Admissions Office to answer nursing related questions and prioritize admission visits to ensure patients with the greatest needs are escalated Makes follow up calls and remote chart reviews to hospitalized patients awaiting discharge to ensure all arrangements are prepared for post-discharge visit including medications, durable medical equipment (DME), transportation, and any other needs Supports Admissions Nurses by ordering equipment, supplies, and medications Available to cover admission and follow up visits based on staffing and referral volumes QUALIFICATIONS: Graduate of an approved school of practical nursing Maintains current Pennsylvania nursing license. One year of nursing experience as a licensed practical nurse within last four years required We offer a full range of benefits including: * Must work a minimum of 64 hours per two-week pay period ^ Must work a minimum of 40 hours per two week pay period
