As a Registered Nurse (RN), you will play a vital role in our care team, working closely with our multidisciplinary healthcare professionals to deliver high-quality, patient-centered care.
Responsibilities:
- Provide individualized patient care and assessment to patients with diverse needs, including those with complex medical conditions
- Develop and implement patient care plans in collaboration with the care team, physician, and family
- Monitor patient progress, update care plans, and communicate with the care team and family about patient status
- Coordinate with various healthcare professionals and community resources to ensure continuity of care and seamless transitions
- Participate in staff development and education, staying current with industry trends and best practices
- Collaborate with our medical director to develop and implement evidence-based care practices
