Nurse-LPN, ACT (24172)

As a Licensed Practical Nurse in the Assertive Community Treatment (ACT) program, you will coordinate medical, dental, and mental health services to adult clients enrolled in the program. You will support the RN and Psychiatric Nurse Practitioner, and be responsible for meeting with clients and aiding in medication administration and additional needs. You will report to the Director of the ACT Program, a Licensed Mental Health Professional, and work onsite and in the community. Review lab and test results, reporting critical values to RN and providers Manage medications, including ordering, obtaining prior authorizations, and dispensing; preparing medication boxes and overseeing client administration Communicate with the Director and ACT team regarding observations in client’s health/behavior Provide client and family education regarding medications and treatments Meet with clients and families within their homes and communities in Saratoga, Warren, and Washington counties Collaborate with the ACT Team during daily team meetings regarding concerns and behaviors Document nursing care using the electronic health record Support admissions and discharges of clients and their medication needs Collaborate with service providers and pharmacies to support client needs Perform other duties as assigned within scope of practice and based on agency needs Requirements And Education LPN licensed in New York State CPR certification required 1–2 years of experience NY‑state license required as travel is required Benefits Health care package: medical, dental, and vision Tuition reimbursement Paid time off Holiday pay 401(k) plan Life insurance Loan repayment assistance Free training/CEU opportunities Northern Rivers Family of Services is an Equal Opportunity Employer.
