Job Description

GENERAL SUMMARY: Lead the coordination and delivery of exceptional, compassionate patient care in our acute dialysis setting. You'll leverage the nursing process—assessment, planning, implementation, and evaluation—to deliver, delegate, supervise, and document comprehensive care and patient/family education. Collaborate seamlessly with the healthcare team to communicate vital patient and family information, and take an active role in identifying and implementing solutions that keep patients on track with their care goals.

KEY RESPONSIBILITIES:

 

• Deliver high-quality, patient-centered nursing care using evidence-based clinical practices and the nursing process
• Assess patient conditions, develop individualized care plans, and evaluate outcomes to optimize patient recovery and quality of life
• Delegate and supervise nursing tasks while maintaining accountability for patient safety and care quality
• Communicate effectively with patients, families, and interdisciplinary healthcare teams to ensure coordinated, compassionate care
• Document patient care accurately and thoroughly to maintain comprehensive medical records
• Educate patients and families on dialysis procedures, self-care management, and health maintenance
• Identify variances from patient goals and collaborate with the team to develop and implement effective solutions
• Demonstrate clinical leadership by fostering a culture of excellence, safety, and continuous improvement

Qualifications

EDUCATION/EXPERIENCE REQUIRED: BSN preferred. Graduation from an accredited school of nursing. Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans. Ability to perform a wide variety of professional nursing services. Ability to effectively communicate with patients, families, and hospital personnel.
CERTIFICATIONS/LICENSURES REQUIRED: Current licensure by the Michigan State Board of Nursing. Health care provider certification (BLS).
