Minimum Requirements

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent entry-level degree in Physical Therapy.
• Current State Physical Therapist license in WV or ability to obtain prior to start.
• Proof of current CPR certification required.
• Current driver’s license, reliable transportation, and vehicle insurance required.
• 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
• Experience with durable medical equipment and ordering rehabilitative equipment required.
• Ability to perform home assessments and adapt equipment to home limitations required.
• Ability to perform prolonged standing, walking, heavy lifting, bending, pushing and pulling required.
• Ability to work flexible schedules and travel locally required.
• Previous home health experience preferred.
• Must read, write, and speak fluent English.

Role Overview and Responsibilities

• Provide physical therapy services to patients across the lifespan in home health settings.
• Conduct clinical assessments and determine appropriateness for services.
• Develop, document, and revise Plans of Care, progress notes, and clinical records in accordance with agency and physician guidance.
• Provide treatment for neuromuscular, neurological, and orthopedic conditions based on referral and evaluation.
• Instruct patients and families on transfer techniques and safe use of mobility and assistive devices.
• Order and adapt rehabilitative equipment to meet patient and home environment needs.
• Apply infection control and universal precautions during patient care activities.
• Collaborate with interdisciplinary teams and accept assignments consistent with education and competence.

Preferred Qualifications

• Previous home care experience.
• Additional certifications or coursework in geriatrics, neuro-rehabilitation, or orthopedics.
