This position will work days M–F at this time.RESPONSIBILITIESEvaluation, diagnosis and treatment of patients with adverse health conditions, and providing education to patients and families within scope of practice and in collaboration with the supervising/collaborating physician.Provide direct primary and acute care services to adult and/or pediatric patients as part of the primary care inter-disciplinary team in the ambulatory departmentPerform initial and on-going assessment of patients’ medical, physical and psychosocial statusObtain and evaluate health and medical history; perform physical exam, conduct preventive screening procedures based on age and historyIdentify medical and health risksComplete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-upFormulate appropriate differential diagnosis based on history, physical examination and diagnostic testingIdentify needs of the individual, family or community and develop a plan of care and/or education planOrder, conduct, and interpret diagnostic tests, therapeutic treatments; may prescribe, administer and dispense therapeutic devices and legend drugsMake appropriate referrals to other health professionals and community agenciesReassess and modify the plan as necessary to achieve medical and health goalsFor the department of pediatrics performs evaluations, including pertinent history and physical exam on patients assigned to include normal newborn babies in the nurseryContribute to on-going medical and nursing staff teaching programsDevelop a patient education plan relevant to the condition and patient/ family needsParticipate in performance improvement activities as assignedAdhere to productivity standards established by the departmentAssess need for personal growth and development and take the initiative in seeking opportunities to meet this needFoster a service that is consistent with the family-centered care and shared management philosophy of the departmentDemonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environmentPromote effective working relations and work effectively as part of a department/ unit team inter-departmentally to facilitate the department’s/ unit’s ability to meet its goals and objectivesPerform other related duties but only after appropriate trainingQUALIFICATIONSMinimum Qualifications:Master's degree in Nursing or in Physician Assistant studies from an accredited programCurrent license to practice as an Advanced Practice Registered Nurse (APRN) or Physician Assistant in the State of Minnesota (or eligibility)NP candidates must have current certification as a Family Nurse Practitioner or ability to obtain within 2 months of offerKnowledge of electronic medical documentation software/systemsPreferred Qualifications:Previous years of experience within Family Medicine as an FNP or PA preferredSALARYNurse Practitioner or Physician Assistant pay is based on year of licensure and specialty area.
