Registered Nurse Clinical Case Manager - Heart and Vascular Services

Registered Nurse Vascular Surgery
Join a high-performing Vascular Surgery team in a dynamic academic medical center environment. This role is ideal for a Registered Nurse (RN) with cardiac, vascular, or critical care experience who thrives in a hands-on, patient-centered clinical setting.
The RN will coordinate interdisciplinary care for complex vascular and cardiovascular patients, supporting patients across the continuum—from clinic visits through follow-up care. This position blends clinical expertise, care coordination, and direct patient interaction in a highly collaborative environment with surgeons and advanced practice providers.
Key responsibilities include RN triage, patient assessment, care coordination, patient intake, insurance authorization, medical record review, and ongoing patient follow-up. The RN will work directly alongside attending physicians in clinic, assisting with wound care, vascular procedures, patient education, and discharge planning.
This is a primarily in-person clinical role with strong emphasis on vascular disease management, cardiac-related conditions, and post-surgical care. Active participation in weekly education and clinical development is expected.
Ideal background includes experience in:
- Cardiac nursing (CVICU, ICU, telemetry, step-down)
- Vascular surgery or vascular medicine
- Wound care and post-surgical management
- Interventional cardiology or procedural areas
- Care coordination or ambulatory specialty clinics
Minimum qualifications:
- Two (2) years nursing experience
- Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
- AHA BLS prior to end of orientation period
Preferred qualifications:
- Previous Vascular Surgery/wound care experience.
- Outpatient experience preferred.
