Job Overview

We are looking for a dedicated physician trained in Internal or Family Medicine to join our team in delivering exceptional patient care. This role involves providing a comprehensive range of primary care services, which include diagnosis, treatment, care coordination, preventive care, and health maintenance.

Minimum of two to three years of relevant outpatient experience is required.

• At least two to three years of experience in primary care within an outpatient environment.

• Proficient in diagnosing and managing a variety of chronic conditions to enhance patient outcomes.

• Exhibit strong diagnostic and treatment skills to uphold high standards of patient care and safety.

• Adopt a patient-centered approach to ensure a positive experience for all patients.

• Exceptional communication abilities.

• Interest in Population Health Management.

Key Responsibilities:

• Assess patient histories and conduct physical examinations. Diagnose and provide appropriate treatment and care.

• Refer patients to specialists when necessary.

• Utilize quality clinical diagnostic skills in patient care delivery. Review test results and recommend appropriate management strategies, including preventive health measures.

• Prescribe medications and develop ongoing treatment and disease management plans.

• Deliver relationship-centered care, addressing both psychosocial and physical patient needs.

• Gather and document patient information, including medical histories and examination results. Ensure medical records are complete and accurate.

• Communicate effectively with the clinical team to ensure comprehensive quality care.

• Engage in medical staff educational programs and meetings.

Qualifications:

• Doctor of Medicine (MD), Doctor of Osteopathy (DO), or equivalent international degree.

• Completion of a three-year residency program in Internal Medicine or Family Medicine from a U.S. accredited institution.

• Relevant work experience, knowledge, skills, and abilities are essential.

• Seeking candidates with outpatient training.

• Board Eligible or Board Certified in Internal Medicine or Family Medicine.

• Must hold a current, unrestricted license to practice in New York State.

• Active Drug Enforcement Agency (DEA) registration or eligibility for registration is required.

• Active Medicare/Medicaid enrollment or eligibility to enroll is necessary.

• BLS or ACLS certification is required.

• Strong verbal and written communication skills are essential. Familiarity with Electronic Medical Record (EMR) systems and Microsoft Office is preferred.
