Clinical Pharmacist

Aspart of East Cleveland Group multi-disciplinary team, the post-holder will playa key role in developing and maintaining clinically and cost-effectiveprescribing provision across the network population - improving patientoutcomes and helping to relieve service and financial pressures across the PCN. Therole will include supported progression through the CPPE Primary Care PharmacyEducation Pathway, providing an exciting opportunity to develop knowledge andskills in relation to patient-facing, person-centred and integrated care. Main duties of the job Thepost holder will be responsible for the care management of patients withchronic diseases undertaking clinical medication reviews to support theproactive management of complex polypharmacy patients. This will include, butnot be limited to, elderly people in care homes, patients with multipleco-morbidities and those with learning disability and autism. Thepost-holder will additionally provide clinical leadership in the delivery ofperson-centred medicines optimisation, helping to support standardisedapproaches to care across the PCN and forging strong relationships with both specialistand community pharmacists to promote system-wide medicines optimisation andquality improvement. The post-holder will provideday-to-day support to practices and will be responsible for supporting andsupervising Pharmacy Technicians. Ideallythe post-holder will be a non-medical prescriber though there will be anopportunity to work towards a prescriber qualification following completion ofthe Pathway and this will be supported by the network as part of continuedprofessional development. About us East Cleveland PCN consists of 5 member practices serving a patient population of approximately 42,000. The successful applicant will be expected to work in any of these practices to support with service delivery. Job responsibilities Ledby a Senior Clinical Pharmacist and working alongside a wider team of ClinicalPharmacists and Pharmacy Technicians across the network, the post-holder willprovide day-to-day support to practices in respect to prescription andmedication queries. They will support the repeat prescription system, deal withacute prescription requests, and medicines reconciliation on transfer of careand systems for safe prescribing providing expertise in clinical medicinesadvice, whilst proactively and reactively addressing both public and socialcare needs of the network population. Person Specification Skills Audit and quality improvement, including the review and analysis of data; risk/issue management and addressing quality/performance in practice Digital skills, including ability to access and competently use clinical systems and electronic transfer of prescriptions Strong clinical leadership, communication and interpersonal skills Ability to apply effective clinical judgement in the prioritisation and decision-making aspects of the role Full driving licence Qualifications Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role Hold or be working toward an independent prescribing qualification (although this will be offered as part of the role) Qualified or enrolled on CPPE Clinical Pharmacist Training pathway (although this will be offered as part of the role) Knowledge and understanding of Primary Care Network arrangements and their contribution to the NHS 10-Year Plan Knowledge, awareness and experience of brief intervention and referral pathways that provide opportunistic advice and support for preventable illness and/or long-term conditions receptive to lifestyle/pharmacological support Experience Minimum 2 years pharmacist experience Experience in conducting medication reviews Experience in developing and implementing new policies and processes to improve the quality and safety of practice, in the context of medicines optimisation Experience in supporting common acute and long-term conditions Experience of working in GP practice Experience in working in a care home setting in the capacity of providing patient-centred medicines optimisation support Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £47,810 to £54,710 a yeardepending upon experience #J-18808-Ljbffr North Thames Genomics Laboratory Hub
