Mobile Case Manager: Empowering Community Wellness

AltaPointe Health Systems in Alabama seeks a Case Manager to monitor consumers’ well-being across mental and physical health, living, learning, working and social environments. You will travel to homes, boarding homes, schools, and other settings to provide monitoring and support, with emphasis on empowering consumers and families to sustain progress after case management ends.
