Youth Case Manager Transitional Living & Outreach

Pivot, Inc. in Oklahoma City, OK is seeking an experienced Case Manager to deliver targeted outreach for homeless youth. You will coordinate with partner agencies, assess readiness, build case plans, and assist with benefits enrollment, housing navigation, and education/vocational goals.
The role emphasizes trauma-informed care, crisis intervention, and data entry into PASIS/HMIS. Requires a 4-year degree, Case Manager II/III certification, and ability to travel up to 50%.
