Licensed Practical Nurse – Nursing

Full Time, 5 shifts of 8 hours each. Rotating shifts including Days, Evenings, Nights, and Weekends. Holiday work required.

1 year of experience as an LPN is required.

Requirements

• Current NY LPN Licensure
• BLS Certification (AHA or RQI only)
• Experience in adult and geriatric patient care
• Ability to assess and interpret patient data reflecting age-specific needs
• Knowledge of nursing principles and practices

Responsibilities

• Collect and communicate data related to residents' physical and psychosocial conditions.
• Collaborate in creating and updating individualized care plans with the interdisciplinary team.
• Participate in evaluating resident outcomes and recommend plan revisions as necessary.
• Maintain clinical competency through ongoing education and mentoring of staff.
• Demonstrate leadership under the supervision of a Registered Nurse.
