Benefits for our Home Health Physical Therapists include: $70-$80 per hour plus mileage reimbursement 1:1 therapist-to-patient ratios, allowing you to significantly influence outcomes Flexible assignments that promote autonomy and a healthy work-life balance Access to online training, professional development, and opportunities to earn CEUs Tuition discounts available through Rasmussen University Paid time off, holiday pay, and comprehensive Medical/Dental/Vision & 401(k) benefits As a Home Health Physical Therapist, your key responsibilities will include: Delivering home-based physical therapy to patients who have experienced a decline in mobility, strength, and functionality due to illness or injury Collaborating with a home health team that may consist of an RN, LPN, CNA, HHA, OT, and SLP, focusing on the patient’s care plan and objectives Monitoring movements, performing assessments, documenting progress, and adjusting care plans as necessary Guiding patients through exercises and hands-on therapy to manage pain and enhance mobility Educating patients about their care plans, established goals, exercises, and self-management techniques Coaching family members on the care plan, therapy objectives, and safe exercise assistance Essential qualifications for Home Health Physical Therapists include: Graduation from an accredited Physical Therapist Program and possession of an active PT license in California At least 1 year of physical therapy experience, preferably in home healthcare settings Current CPR certification Familiarity with state and federal home health regulations Strong clinical judgment, excellent interpersonal skills, dexterity, and compassion Why join our organization?
