GENERAL DESCRIPTION OF JOB: The Case Manager II provides comprehensive assessment, case management, treatment planning, restorative justice facilitation, and participant support throughout the diversion process. The position works with participants, families, internal team members, and community partners to support successful completion of individualized diversion plans. Key functions include conducting assessments and screenings, coordinating services, maintain participants’ records, monitoring progress, participating in multidisciplinary meetings, identifying and fostering professional relationships with other community-based providers, connecting diversion participants with community resources and providers as needed, providing educational advocacy and supporting restorative justice practices. Duties may include clinical assessment and therapeutic services when the employee meets required qualifications. DUTIES: Coordinates with the diversion team to identify appropriate restorative justice practices for participants and facilitates restorative justice practices including community circles. Collaborates with clinical staff to create individual plans outlining program requirements for both adult and juvenile diversion participants. Ensures appropriate level of supervision for each client, determines frequency and modality of participants contacts with the program. Works in conjunction with program participants to promote successful completion of their individualized program goals. Coordinates shared treatment planning with external treatment providers, executes appropriate waivers and releases of information, and ensures compliance with treatment and program requirements. Researches and identifies community-based resources as needed, conducts due diligence on community-based resources and establishes professional working relationships between agencies to foster ease of access and collaboration. Conducts multi-disciplinary meetings. Helps families to understand educational options and coordinates with schools on transitions. Educates client and family regarding the justice system and consequences of further law involvement. Assists in tracking and collection of restitution. Creates case management notes in participant electronic files and all other required paperwork or documentation, maintains participant confidentiality, tracks participant progress and helps to determine program violations and successful completion. Collects and reports various case-related statistics. May assist with data collection and reporting related to grant reports and applications. Formulates case conceptualization and case presentations to supervisor, co-workers and outside agencies. Completes screens and assessments to determine risk and need levels and to guide treatment planning. Completes assessments, intakes, and risk/need evaluations. Provides therapeutic services such as individual, family, group, couples, or experiential counseling (based on qualifications). Incorporates victim input into participant planning. Participates in staff meetings, supervision, retreats, trainings, and conferences. Delivers presentations to schools, agencies, and community partners.

REQUIREMENTS:

Skills, Abilities and Competencies: Familiarity with professional and clinical emerging knowledge, and an interest in the criminal and juvenile justice systems and trends. Ability to coordinate with community agencies/resources, facilitate meetings with outside providers on behalf of the client, understand and explain program components to clients, families and outside agencies, identify and analyze problems, make recommendations for effective solutions, establish and maintain positive working relationships. Effective oral and written communication, ability to multi-task, self-driven and accountable to supervision. Works as a member of a high-functioning, self-directed team. Exceptional time management and organizational skills. Knowledge of restorative justice principles and experience facilitating varied restorative justice practices. Behavioral Competencies (these are required for all positions at ACG): Accountability Accessibility Inclusivity Integrity Education and Experience: Master's degree required in social work or closely related human services field. Licensure, or candidate for licensure, in a mental health filed per Colorado DORA. 2+ years of progressively responsible case management duties involving direct service provision with adults, juveniles, and families. Any equivalent combination of related work experience and education will be considered. Preferred Education and Experience: Bi-lingual (Spanish-English) preferred Post Employment Requirements: Security clearance required: will require fingerprinting and a Colorado Bureau of Investigation (CBI) background check. Pre-Employment Additional Requirements: Must successfully pass pre-employment testing which includes an acceptable motor vehicle record (MVR) and background check. Possession of a valid Class “R” Colorado Driver’s License or the ability to obtain one within two weeks of appointment. WORK ENVIRONMENT:

The majority of work is provided in a standard office environment. Opportunities to provide community service, day and overnight experiential therapy, restorative justice and various other modalities are incorporated.

PHYSICAL DEMANDS:

Spends 90% of the time sitting and 10% of the time either standing or walking. Occasionally lifts, carries, pulls or pushes up to 20 lbs. Occasionally uses cart, dolly, or other equipment to carry in excess of 20 lbs. Occasionally climbs, stoops, kneels, balances, reaches, crawls and crouches while performing office or work duties. Verbal and auditory capacity enabling constant interpersonal communication through automated devices, such as telephones, radios, and similar; and in public meetings and personal interactions. Constant use of eye, hand and finger coordination enabling the use of automated office machinery or equipment. Visual capacity enabling constant use of computer or other work-related equipment.
