Hybrid PCP Case Manager - Person-Centered Planning

Ardmore Enterprises is seeking an Internal Case Manager (PCP Specialist) in Maryland. You will develop and monitor Person-Centered Plans, coordinate with families and staff, and ensure regulatory compliance while advocating for individuals we support. The role requires a Bachelor's in Human Services, 3–5 years of case management, a valid driver’s license, and strong computer skills.
