Rounding RN: Hospital-Clinic Liaison & Care Coordinator

Community Health Network

Community Health Network in Indianapolis, Indiana is seeking a Rounding Registered Nurse to act as a liaison between the physician office and hospital team. In this role, you will conduct hospital rounding, educate patients, triage calls, and coordinate care throughout the discharge process. The ideal candidate will possess a nursing degree from an accredited institution and a valid RN license in Indiana. At least three years of nursing experience is required, with a preference for cardiology and outpatient experience. #J-18808-Ljbffr Community Health Network
