Opportunity Highlights:
- Full-time Primary Care NP/PA position in an established PACE program
- Care for an older adult population using a collaborative interdisciplinary team model

- Coordinate care with physicians, specialists, therapists, and nursing teams

- Rotating evening and weekend call

Compensation & Benefits:
- $150K–$190K salary plus annual bonus

Qualifications:
- Licensed California Nurse Practitioner or Physician Assistant
- Minimum 2 years of NP/PA experience
- Experience caring for older adults preferred
- Current prescriptive authority and DEA (or eligibility)
