Position Overview

This position involves coordinating and directing home nursing care according to physicians' orders, while adhering to agency policies and procedures through the effective application of the nursing process. The role includes supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Responsibilities

• Provide comprehensive home nursing care based on physicians' orders.
• Supervise and evaluate the performance of Home Health Aides and LPNs/VN.
• Conduct home visits and maintain accurate patient documentation.
• Participate in periodic office meetings.
• Ensure all care is delivered competently and compassionately.

Requirements

• Minimum of 1 year of experience in Adult Home Health or Hospice care, depending on specialty.
• Current CPR and BLS certifications required.
• Must hold a valid nursing license in the state of practice.
• Proficiency in the HomeCare HomeBase (HCHB) electronic medical record system is necessary; candidates must have used it within the last 6 months or be willing to complete a retraining course.
• Proficient in OASIS documentation for RNs, with recent usage required.
• Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Willingness to travel up to 75 miles from the assigned care center.

Work Schedule

• Shift: Day 5x8-Hour (08:00 - 17:00)
• Contract assignment duration: 13 weeks
• Potential for rotating on-call duties with other nursing staff.
• Patient assignment is expected to be between 5-10 patients per day.

Additional Information

• Home office located in Roseburg, OR.
• The mileage for travel between patient homes will be reimbursed at the IRS standard rate.
• Weekly productivity points expectation: 30 points per week, equating to a minimum of 40 hours of work.
