Dual-Certified Nurse Practitioner (FNP/PMHNP)***PRN***

Location: Chicago, IL Job Type: Part-Time / PRN What You’ll Do As a dual-certified Nurse Practitioner, you will provide comprehensive, whole-person care across medical and psychiatric domains in skilled nursing and post-acute settings. Post-Acute / Primary Care Responsibilities Perform comprehensive physical exams and review medical histories Diagnose and manage acute and chronic medical conditions Order and interpret diagnostic testing (labs, imaging, etc.) Prescribe medications and develop individualized treatment plans Collaborate with physicians, nursing staff, and interdisciplinary teams Educate patients and families on disease management and prevention Document clinical encounters accurately in the EHR Conduct psychiatric evaluations, follow-ups, and consultations Assess patients for mental health conditions and behavioral concerns Develop and implement individualized treatment plans Evaluate need for higher levels of care, including hospitalization Provide medication management and monitor treatment adherence Deliver patient and family education related to psychiatric care Work closely with supervising physicians and care teams to optimize outcomes Maintain strict confidentiality and high standards of client-centered care Who Should Apply?
