You’ll work closely with patients, caregivers, and their care teams to improve mobility, strength, independence, and quality of life. Your responsibilities may include: Providing comprehensive physical therapy evaluations Developing individualized treatment plans based on each patient’s needs Providing therapeutic exercises and functional mobility training Assessing patients’ home environments and recommending appropriate safety modifications Educating patients and caregivers Monitoring patient progress and modifying treatment plans as needed Collaborating with physicians and other members of the care team Completing accurate and timely electronic documentation Home care offers a unique opportunity to get to know your patients and make a meaningful difference in their daily lives.
