Work Shift Day (United States of America) Job Summary The Wound, Ostomy, and Continence Clinical Nurse (WOCN) functions under the direction of the Nurse Manager and provides acute and rehabilitative care for people with select disorders of the GI, GU and Integumentary systems. The WOCN serves as a resource to nursing and medical staff and is responsible for delivering in‑depth, comprehensive patient care in complex multiple‑problem patient care situations. The WOCN can independently perform job functions and is a licensed caregiver who upholds the voice of the patient, system policies and procedures while supporting service excellence goals in a shared governance model. Clinical practice is executed with best evidence through continuous learning from formal education, active reading of professional literature and reflective practice. The WOCN serves as a partner in care, anticipating and providing for the patient care needs of patients, families and significant others. Role‑specific competencies, responsibilities and practice are guided by the WellStar Professional Practice Model and the 5‑Star Nursing standards. It is expected that the WOCS nurse is certified, licensed, knowledgeable and practices nursing in accordance with the Georgia Professional Nurse Practice Act and the Scope of Practice and Code of Ethics Standards of the American Nurses Association. Core Responsibilities And Essential Functions Act as a consultant to the physician, providing evaluations and developing individualized, evidence‑based plans of care that address wounds, fistulas, ostomies and continence issues at the specialist level. Respond to the physician consult request by writing orders within the WOCN scope of practice, determining the type and frequency of wound/ostomy care, including modifications as the patient’s condition changes. Upon receiving a physician order for consult, mark patients for ostomy surgery, select the best and alternative sites for the surgeon, provide education to the patient and family pre‑ and post‑op, and guide psychosocial, sexual and body‑image adaptation to promote optimal outcomes. Perform the Nursing Process (assessment, diagnosis, outcomes, planning, implementation and evaluation) using evidence‑based practice and analytical/critical thinking to ensure individualized care. Provide relationship‑based, patient‑centered care by partnering with patients and significant others, using comprehensive hand‑off communication and interdisciplinary evaluation, just‑in‑time and planned patient teaching, keeping patient goals at the focus of the plan of care. Practice current clinical standards with a focus on safety and quality. When performing duties in outpatient wound care and hyperbaric units: Demonstrate knowledge of healing strategies and HBO therapy indications, deliver care pre‑, during and post‑treatment, monitor gas delivery, monitor patients for symptoms of oxygen intolerance, claustrophobia, hypoglycemic reactions or other adverse events, intervene under program direction, educate patients and families on HBO therapy, perform ordered treatments (vital signs, blood sugars, emergency procedures) and document each treatment accurately. Maintain chamber operations: apply knowledge of hyperbaric chamber operations to ensure safety, operate controls to pressurize/depressurize according to policy, keep accurate logs of pressure, depth and time, perform maintenance of chambers and support systems, clean chambers after each treatment, and maintain emergency equipment and supplies. Serve as a clinical expert in hospital committees and community groups; lead outcome evaluations, develop product formularies and algorithms, facilitate bariatric population needs and coordinate safe, quality patient care through interdisciplinary communication and respectful, inclusive environments. Participate and support performance improvement initiatives, including research, data collection, and evidence‑based practice activities, while upholding system policies and standards such as TJC Patient Safety Goals and WellStar Health Systems safety absolutes. Precept specialty students and nurse practitioner students, provide education and orientation to new hires and continuing education to existing nurses and staff, and promote a healthy work environment. Ensure patient, family and staff confidentiality, comply with WellStar Health System policies, and serve as a preceptor or mentor to professional nurses and staff. Perform other duties as assigned and comply with all WellStar policies and codes of conduct. Required Minimum Education Associates Nursing or Diploma (Nurse) Nursing or Bachelor’s Nursing – Preferred Required Minimum License(s) And Certification(s) All certifications are required upon hire unless otherwise stated. BLS – Basic Life Support, or ARC‑BLS – American Red Cross Basic Life Support, or BLS‑I – Basic Life Support Instructor RN – Registered Nurse (Single State) or RN‑COMPACT – Multi‑State Compact Nurse Additional License(s) And Certification(s) National certification in area of specialty. Upon hire preferred PALS – Pediatric Advanced Life Support. Upon hire preferred PALS‑P – PALS Provisional within 180 days. Required Minimum Experience Minimum 3 years as a staff nurse in an acute care setting. Required Minimum Skills Ability to read, write and speak English, use technology to support clinical care, hold basic computer skills, strong interpersonal and collaborative skills, customer service skills, ability to function in a fast‑paced environment, respond to emergencies with a decisive and respectful manner, excellent time management, evidence‑based thinking, and uphold the scope of practice and ethical standards of nursing.
