Infectious Disease IV Coordinator - Full Time

The primary responsibility of the ID IV Coordinator is to support the Infectious Disease Department's patients discharged on IV antibiotics. The ID IV Coordinator role involves identification of patients of Infectious Disease physicians discharged on IV antibiotics, review of daily labs, confirmation of office appointment follow-up, and communication of medication changes with outside agencies. The ID IV Coordinator plays an important role with patient safety, particularly in the prevention of medication toxicity and readmissions. The ID IV Coordinator position requires strong communication and interpersonal skills and self-initiative in completion of tasks with minimal direct supervision. Role Overview: What is OPAT?

• Definition: OPAT stands for Outpatient Parenteral Antimicrobial Therapy.
• Clinical Focus: Infectious Disease doctors manage patients with severe, treatment-resistant bacterial, fungal, or viral infections using high-risk intravenous medications.
• The Impact: Historically, these patients required up to a 90-day hospital stay. OPAT allows them to receive their IV therapy at home or in a facility via a central or midline.
• Risk Management: Because these medications carry severe, potentially fatal long-term risks, close monitoring is critical to patient safety.

Key Responsibilities

• Clinical Monitoring: Coordinating weekly lab draws and tracking results to ensure patient safety metrics remain within safe limits.
• Clinic Coordination: Ensuring patients come in weekly to evaluate their central lines, check insertion sites, and assess infection site healing.
• Administrative Oversight: Managing prior authorizations and confirming medication availability with community pharmacies or locations.
• Continuum of Care: Acting as the primary phone contact to coordinate care between providers, community partners, patients, and families.
• Workflow Evolution: Participating in upcoming clinic workflow optimization projects.

Candidate Preferred Skills

• Education: Bachelor of Science in Nursing (BSN) is required.
• Licensure: Registered Nurse (RN) is mandatory. Candidates must be licensed to pull central/midlines, which is outside an LPN's scope of practice.
• Experience Target: Ideally 2+ years of healthcare experience.
• Alternative Experience: Will accept 1 year of experience if it includes specialized exposure to PICC/central/midlines, case management, social work, or prior authorizations or other similar niche skills.
• Soft Skills: Exceptional organizational skills, strong phone etiquette, high emotional intelligence (EQ), and provider communication skills.

• Team Structure: You will join a supportive, tightly knit 3-person team that balances clinic and administrative duties.
• Patient Connection: While this is not hands-on clinical care, the coordinator builds deep, impactful relationships. Our coordinators receive the most appreciation and thank-you cards in the clinic.
• Training: We offer extensive onboarding. Because of the nuances, it takes about a year to fully master the role, and we provide dedicated professional educators to guide them.
• Volume: The team actively manages roughly 120 OPAT patients at any given time.
• Schedule: Currently a standard 5-day work week, but we are actively looking into transitioning this to a rotating 4-day schedule.
• Compensation: This is a salaried position.
