Overview Advanced Practice Clinician, Integrated Care Delivery (Maricopa County) — patient‑facing role covering a defined geography within Maricopa County. SCAN hires for this role on a rolling basis; we will reach out when a position becomes available. SCAN is committed to keeping seniors healthy and independent and values innovation in senior healthcare. Responsibilities Provide and coordinate primary care to SCAN members in their residence, most commonly in senior living communities or skilled nursing facilities. Conduct physical/wellness exams and assessments; perform occasional urgent medical visits and telephonic triage to ensure timely, appropriate care and reduce hospitalizations when possible; administer vaccines; order and interpret diagnostic tests; initiate goals of care conversations with patients and loved ones; manage chronic diseases. Identify health care needs and develop a comprehensive plan of care for the patient, their family, and living‑community staff. Collaborate with the case management team to ensure timely access to necessary services. Independently manage visitation schedules in line with practice protocols and adjust based on member needs and acuity. Engage with Primary Care Physicians and other health care professionals to support disease prevention, health promotion, and chronic disease management; coordinate clinical oversight and guidance with PCPs. Coordinate with specialists, hospitals, ancillary care providers, and community organizations to ensure continuity of care and shared decision‑making while advocating for members. Monitor and address clinical quality gaps in care in partnership with care providers. Document member visits and interactions in a timely manner per practice protocols. Serve as a direct resource for members, families, communities, and support staff; participate in Integrated Care Sales events to build member awareness. Initiate and complete virtual health visits to promote accurate triage and reduce drive time; participate in after‑hours on‑call program. Maintain professional knowledge through educational activities and professional reading. Contribute to team efforts to meet quality goals, close care gaps, and optimize clinical workflows and protocols. Adhere to patient‑facing role expectations and maintain HIPAA compliance. Travel 75+% of the time within the assigned area; valid driver's license and reliable transportation required. Your Qualifications Master’s degree in Nurse Practitioner or a Master’s from an accredited PA program (adult/geriatric focus preferred). Current Arizona Nurse Practitioner license or Arizona PA-C in good standing; equivalent combinations of education/experience may be considered. Wound Care Certificate and Palliative Care Certificate a plus. Knowledge of treatment for complex chronic diseases (e.g., dementia, diabetes, heart failure, COPD). Ability to identify health care needs and coordinate with other resources. Knowledge of HCC and ICD-10 coding and documentation. Knowledge of CMS guidelines and Medicare Advantage Managed Care. Comfort with initiating goals of care and end‑of‑life discussions. Strong interpersonal skills and ability to build relationships with health care teams and patients. Knowledge of HIPAA requirements; adaptable and open to change and problem‑solving. Proficiency with basic technology (Word, Excel, PowerPoint, Outlook, EMR) and openness to new technology. Ability to stoop, kneel, crouch, bend, reach, lift (regularly up to 10 lbs, occasionally up to 50 lbs). Multilingual/multicultural skills are a plus. Travel 75+% of the time in the assigned area; valid driver’s license, automobile insurance, and reliable transportation required. Policies and Benefits Tuberculosis screening: offer contingent on hire; provision of necessary information and accommodations as applicable under law. Base salary range: $125,400.00 to $179,300.00 annually; quarterly bonus program; robust wellness program; generous PTO; eleven paid holidays plus one floating holiday; 401(k) with employer match; tuition reimbursement.
