• Facility Specific Job Notes:

The job requires a variety of credentials, licenses, certifications, and other professional and personal health documentation. Key requirements include: - **Licensure**: Verification through Nursys and License/Certificate Verification. - **Professional References**: Two professional references are required. - **Health and Safety**: Completion of various health screenings and vaccinations including MMR, Hepatitis B, Influenza Vaccine, Varicella, and Tdap. A 2-Step PPD or IGRA is also required. - **Professional Onboarding Requirements**: Education and employment verification, criminal background check, Ascension's core competencies and specific checklists, and other onboarding requirements. - **Personal Identification**: Driver's License or State ID. - Optional personal and professional documentation may include name change documents and some health history records. Note: Some documentation is required for submission, while others are needed for onboarding.
