Hybrid Case Manager - Reentry & Behavioral Health

Community Connections

Community Connections in Washington, DC is seeking a Case Manager to join the ReEntry program, a grant-funded behavioral health initiative for adults transitioning from incarceration to community living.

You will provide intensive, trauma-informed case management, coordinating healthcare, housing, benefits, employment supports, and community resources to support successful reintegration.
