Job Details
- Weekly Estimated Pay: $2,030 - $2,190
- Contract Duration: 13 weeks (April 27, 2026 - July 24, 2026)
- Hours: 40 hours/week (8-hour shifts, Monday-Friday, 8:00 AM-4:30 PM)
- Setting: Home Health (travel physical therapist / home health PT)
- Coverage Areas: Marion County and Hamilton County, Indianapolis metro
- Guaranteed Hours: 40 per week
- Mileage Reimbursement: Yes, between visits (from first patient to last patient)
- On Call/Weekend Requirement: None
- Bachelor's degree in Physical Therapy (minimum requirement) or equivalent
- Active and valid Physical Therapist (PT) license in the applicable state
- Minimum 1 year of home health experience (home health PT experience preferred)
- Experience with EMR/EMAR systems (HCHB preferred)
- Strong communication and patient education skills for community-based care
- Professional certification required (e.g., current BLS or other required certifications)
- Provide comprehensive home health physical therapy services including start of care (SOC), resumption of care (ROC), and discharges
- Assess mobility, ADLs, safety, and functional needs; develop and implement individualized treatment plans
- Oversee and collaborate with Physical Therapist Assistants (PTA) and interdisciplinary care teams
- Ensure timely, accurate documentation and billing using EMAR/EMR systems (HCHB experience a plus)
- Meet productivity expectations (30 points per week) while delivering compassionate, patient-centered care
- No weekends or on-call shifts-consistent Monday-Friday schedule
