Homeside Rehab is seeking a licensed Speech-Language Pathologist (SLP) for home care opportunities in Long Island, NY.

We are looking for compassionate and dependable clinicians who are passionate about providing high-quality, one-on-one care while helping patients improve communication, speech, language, cognition, and swallowing skills in the comfort of their homes.

• Evaluate patients and develop individualized treatment plans

• Provide skilled speech-language therapy based on each patient’s needs and goals

• Monitor patient progress and adjust treatment plans as appropriate

• Complete accurate and timely documentation

• Communicate effectively with office staff, patients, and caregivers

• Collaborate with the clinical team to ensure coordinated and effective treatment

• Work with healthcare professionals, including speech therapy assistants, nurses, physicians, and other members of the care team, to support comprehensive patient care

• Job Type: Full-Time, Part-Time, and Per Diem
