Nurse Practitioners see patients independently and collaborate with physicians, depending on the patient’s specific situation. Nurse Practitioners provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patients understand and feel connected to their care plan.\n\nNurse Practitioners practice in our outpatient practices on a collaborative care team composed of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. We partner with a network of elite specialists and hospitals for specialty and acute care. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health.\n\nCore Responsibilities:\n\n Provision of exceptional primary care.\n Conduct office visits for routine and acute issues.\n Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.\n Care coordination with other providers, specialists, testing facilities, and agencies.\n Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years)\n Assisting the care team with phone triage and outreach.\n Educating patients on their health conditions, care plans, and treatments.\n Participating in Oak Street Health promotional activities.\n Conducting home visits as needed.\n Other duties, as assigned.\n\nThis role reports to the Center Medical Director and works closely with operational leadership.\n\n\nRequired Qualifications:\n\n\n Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate\n National certification in at least one of the following specialties:\n \n Family Nurse Practitioner\n Adult-Gerontology Primary Care Nurse Practitioner\n Adult Nurse Practitioner\n Gerontological Nurse Practitioner\n \n Active, non-probationary state Nurse Practitioner license\n Active DEA license\n US Work Authorization\n Bilingual proficiency in Spanish\n\n\nPreferred Qualifications:\n\n Experience in primary care: internal medicine, geriatrics, or family medicine\n Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).\n Passion for teamwork and the opportunity to collaborate cross-functionally\n Desires to be a part of an innovative model focused on empirically-guided population health\n\n\n\n\n\nAnticipated Weekly Hours40\n\n\nTime TypeFull time\n\n\nPay Range\nThe typical pay range for this role is:\n$104,442.00 - $224,952.00\n\nThis pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.

The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
