The role guarantees 36 hours per week, though the expected caseload includes visiting 5-6 patients daily.Key ResponsibilitiesProvide one-on-one therapy to home health patients, ensuring high-quality care and satisfaction.Manage a daily caseload of patients, which may include visiting up to 6 individuals per day.Complete timely documentation and communicate effectively with the healthcare team.Utilize provided documentation tools and ensure their return post-use.Experience RequirementsMinimum of 1 year of home health experience preferred.Experience with a range of patient conditions, including but not limited to post-operative recovery, cognitive disorders, and rehabilitation needs.Certifications and LicensesCurrent Basic Life Support (BLS) certification required.Must hold a valid Physical Therapist Assistant license in California.Additional NotesCandidates should have their own transportation as the role may involve travel within a 50-mile radius.
