RN Care Specialist

Role: Registered Nurse responsible for assessing residents' health, developing and updating care plans through interdisciplinary collaboration, and ensuring safe delivery of nursing interventions. Key Responsibilities Conduct comprehensive assessments of residents' health, educational, and environmental needs upon admission and regularly thereafter. Communicate significant health changes to the interdisciplinary team (IDT). Develop, review, and revise personalized care plans involving physicians, residents, families, and other team members. Document assessments and care plans accurately, ensuring compliance with regulatory standards. Implement nursing interventions such as medication administration, treatments, and IV therapies safely and promptly. Utilize critical thinking to respond to changes in residents' conditions and notify physicians using SBARR communication method. Monitor residents' care delivery effectiveness, identify issues, and participate in QAPI processes for problem resolution. Assist with activities of daily living, answer call lights, and support resident well-being. Maintain current knowledge of best nursing practices and organizational policies. Qualifications & Skills Graduate from an accredited nursing program with RN licensure in Missouri & Illinois. Current CPR/AED certification. Experience in long-term care and rehabilitation settings. Strong critical thinking, communication, time management, and organizational skills. Physical ability to stand, walk, lift, and assist with residents and equipment, including heavy objects. Manual dexterity, good visual acuity, and proficiency in electronic and written documentation. Willingness to work across multiple locations and travel as needed.
