Our Home Health RN is responsible for providing individualized goal-oriented nursing care using the nursing process in accordance with a physician's order. In addition, this position serves as Case Manager in cases in which this position is the lead discipline. COVID vaccination required - no exemptions. Oasis experience is a must. Home health experience is required. Experience as a home care traveler with proficiency in OASIS is preferred, as this role includes case management responsibilities. Homecare Home Base experience is preferred.

Shift: DAYS: Monday-Friday (with weekend rotation approximately every 4th week)
