Reporting to a supportive Rehabilitation Services Manager, the Long Term Care Physical Therapist gains day-to-day backing from clinical educators, therapy techs, and rehab aides, while partnering closely with friendly nurses, case managers, social workers, OTs, SLPs, PTAs, and physicians; this diverse, collaborative team ensures every Registered Therapist can practice at the top of license with the right tools, consults, and communication channels to keep patient care seamless.

Responsibilities for the Long Term Care Physical Therapist span comprehensive evaluations, individualized treatment plans, gait and balance training, neuro and orthopedic interventions, restorative nursing collaboration, caregiver education, discharge and transition planning, accurate EHR documentation, and outcomes tracking; evidence-based protocols, decision support, and quality-improvement initiatives set a Licensed Physical Therapist up for success, with clear pathways toward clinical specialization, lead roles, and teaching or precepting opportunities.
