Job Summary:

Modani Care, a leading provider of in-home care services, is seeking a compassionate and dedicated Home Health Caregiver to join our team in McRae-Helena, GA. As a Home Health Caregiver, you will play a vital role in delivering personalized care and support to seniors and adults who need assistance with daily living.

Responsibilities:

• Provide high-quality care and support to clients in their homes, focusing on their physical, emotional, and social needs
• Assist with daily living activities such as bathing, dressing, grooming, and toileting
• Assist with mobility and transfers, including walkers, canes, and wheelchairs
• Provide medication reminders and assistance with medication management
• Collaborate with our interdisciplinary care team to develop and implement individualized care plans
• Promote independence, dignity, and well-being of clients, while encouraging social interaction and engagement
